Making a Difference Since 2009

We’ve designed and delivered community-centered food and health programs across Chicago and Southern Cook County -grounded in culture, equity, and Black community leadership.

2009–2016

2017–2019

BUILDING THE FOUNDATION

Community Markets Across Chicago

  • 900–1,000 people served biweekly at farmers' markets across Chicago

  • 6 community locations from Auburn Gresham to South Shore

  • 20 tons of fresh food sold

  • 30–80% sourced from Black farms in Chicago and Pembroke Township

Partnering with Healthcare

Cook County Health 

  • 20+ Community Health Centers served across Southeast Chicago

  • Developed Food as Medicine prescription system with Cook County Health

  • Health screenings, cooking demos, and provider partnerships

Crisis Response

Feeding Communities in Crisis

  • 60,000+ Local Farm to Family Food Boxes distributed across 13 locations

  • 3,000+ Rx Bags funded by community donations

  • Launched online ordering and delivery

Scaling Impact

  • 7 tons in 2024 → 30+ tons in 2025 supplied to Southern Cook County

  • $1 million in Illinois-grown food purchased (40% from Northeast Illinois)

  • Food as Medicine Hypertension Community Intervention: 250 households across 4 Southern Cook County Communities (Phoenix, Roseland, South Shore, Robbins) saw reduced blood pressure and improved well-being

  • Reconnected Pembroke Township farmers with descendants of Great Migration families

  • Lead Agency for IL EATS program

  • Healing Fellowship: Trained 3 food entrepreneurs in health-focused meal prep.
